Steps in Converting a PDF to a Fillable Form
Go to the form builder dashboard in PlatoForms. If you do not have an account yet, you can create an account, or if you have an existing account, you can click login. And you will be directed to your dashboard.
Once you are directed to your dashboard, look at the menu on your left side.
Step 1: Find the button or option “Start to build” from the left menu navigation pane.
Then after clicking that, a pop-up box will appear. You will see 3 options, namely:
- Create an online form existing PDF; this will allow you to upload a PDF file that will be your basis for the PDF form. Only one PDF file is suitable for this form.
- Create a Master form; this is used for multiple PDF Forms and
- Create a PDF from scratch; it means that you will create or build your own form since you haven’t built one before.
Step 2: Then select click the create an online form to start then uploading your file.
After that, you can select what type of form you want to build. The page will give you two options:
- Online Web Form – this is a shareable and fillable form that you can generate and edit and share with anyone using a link.
- Online PDF Form - will keep an intact PDF format accessible to Firefox, Chrome, Safari, etc. Once you are done choosing, you can now edit, convert, design, and publish your work.

How to Design your Form?
Once you open the file, you can notice that above the form that there are tabs which you can use the first one is:
ADD - This tab will show the Form and PDF Attributes.
You can find attributes in the form, namely, text input, dropdown, signature, and photo. Which means you can add and format these items in your form. While PDF Attributes contain elements like font style and size.
To move and resize the objects or the boxes in your form, you can click and drag it for adjusting, positioning, or resizing.
How to use the Preview Options?
Click the preview tab for a preview of your work. You can choose the preview form, and it will show you the form design and preview with blank details. While Preview PDF will let you view a file wherein it has filled-up data, and that will be what it looked like when someone entered his or her data in there.
How to Publish your Work?
If your form is already okay for you, click publish. You will go back to your dashboard a pop-up box will appear, asking you to choose a public or embedded script. Please copy the link that you chose and share it with the people or participants of your research or survey.
How to Monitor your data?
Information gathered from your participants will be sent to your email for monitoring. You can also check the Submissions on the left side of the Menu.
