Ask for the right documents every time

Name each document and add instructions, make it required or optional, and allow up to 10 files for items like the front and back of a card. Limit uploads to images and PDFs, or PDFs only, and set a size limit. Start from a ready-made checklist for patient intake, new clients, new hires, or vendors, or let AI suggest one from your form’s fields.

After the form is submitted, the checklist appears on the thank-you page, and the person also receives a Documents link by email or text message. No account is needed. Reminders go out on the days you choose until every required file is in. Add a passcode or an expiry date to the link when the documents are sensitive.

Your team is notified when files arrive. Accept a file, or reject it with a note, and the person is asked to upload it again. Accepted files can be filed to Google Drive, Dropbox, Box, or OneDrive automatically. Uploaded images are re-encoded and PDFs are checked for active content before anyone opens them.

Set when each document expires: a number of months after upload, the date in a form field, or a date the reviewer enters when accepting it. How document requests work

  1. Add a checklist to your form

    Open your form’s settings, turn on Document requests, and list the documents you need. Choose who reviews them and when each one expires.

  2. Share the form as usual

    Use a link, an invitation, or an embed on your website. Nothing changes for the person filling it in until they submit.

  3. Collect, remind, and review

    The person uploads each document from their link. Reminders cover anything missing, your team accepts or rejects each file, and the request is complete when every required document is accepted.

  4. Renew before anything expires

    Accepted documents keep their expiry dates. Ahead of each date, the person is asked for a new copy or to renew the form, and your team sees what is due, sent, and expired in one list.

Questions about document requests

What to know before you ask for your first documents.

1. How do I request documents from clients after they fill out a form?
Turn on Document requests for the form and add a checklist of the documents you need. When a client submits the form, PlatoForms shows the checklist on the thank-you page and sends them a Documents link by email or text message, where they upload each file without creating an account. Reminders continue until every required document is in.
2. Can I track when documents expire?
Yes. Each document can expire a number of months after upload, on the date in a form field, or on a date your reviewer enters when accepting it. With renewals turned on, PlatoForms asks the person for a new copy before the date, sends the reminders you choose, and marks the document expired if nothing arrives. Renewals are included from the Silver plan.
3. Can a whole form be renewed every year?
Yes. Set a submission to expire a number of months or years after it was submitted, or on a date in the form. Before it expires, the person receives the form again with their last answers filled in, and you can lock answers that should not change. When they resubmit, the renewal is closed and the next one is scheduled.
4. Do people need an account to upload their documents?
No. Each person uploads from their own Documents link. For sensitive documents you can require a passcode and make the link expire after a set number of days.
5. Is document collection HIPAA compliant?
Yes, on a HIPAA-enabled Silver plan or higher. Uploaded documents are stored with the form’s submissions, Documents links expire after 30 days by default, and a passcode is on by default. See HIPAA-compliant online forms.
6. Do uploaded documents count toward my storage?
Yes. Uploaded documents use your plan’s storage, the same as files uploaded in a form. If storage is full, new uploads are paused until space is freed or the plan is upgraded.
7. Can I connect document requests to other tools?
Yes. Webhooks report when documents are requested, received, reviewed, and complete, and when renewals are scheduled, sent, completed, or expired. Requests and renewals are also available in the PlatoForms API and the MCP server for AI assistants.
8. How does PlatoForms compare with Content Snare?

Documents follow a form you already use. In PlatoForms the checklist belongs to your form, so the answers, the documents, and the filled PDF stay in one submission. Content Snare sends its own requests made of questions and file fields.
