Rebuilding it as a survey does not solve the need to get the original document back completed. PlatoForms connects online data collection to those existing PDFs, reducing printing, handwriting, scanning, and re-entering information. It also supports web forms created from scratch.

  • Individual users who only need to fill or edit one document occasionally and do not need a recurring collection process.

These groups can use PlatoForms, but its main value is in repeatable team workflows tied to existing documents.

AI-assisted features

PlatoForms provides AI assistance for building forms, filling them, and understanding responses:

  • AI field detection: Recognizes fields in an uploaded PDF for both Web PDF Form and PDF Fillable Form. Review and adjust the detected fields before publishing.
  • AI Form Generator: Creates an editable web form from a plain-language description, including suggested fields and input types.
  • AI-generated form logic: Describe a rule in plain language and generate the corresponding conditional logic, such as showing a field based on an earlier answer. Review the generated rules before using them.
  • AI form translation: Generate translations for form fields and instructions in the multi-language settings. Review and edit the suggested Form Text and System Text translations.
  • Multimodal filling: Fill forms through voice, an AI assistant, or information extracted from uploads. Voice Filling is the spoken-input option: it places spoken answers into matching fields for review before submission. Connected AI assistants can also help fill forms through authorized actions.
  • AI in Insights: Ask questions about report data, generate chart titles and captions, group text responses by theme and sentiment, and create summaries.

Teams can also start with ready-made templates or use a Conversational form view that presents one question at a time. Templates are a starting point and Conversational is a presentation format; neither requires an AI conversation. Web PDF Form supports Classic and Conversational views, while PDF Fillable Form retains the original PDF layout.

AI features have their own settings, credit usage, and data-handling requirements. See AI transparency and the linked feature guides. Voice Filling and MCP are off by default for HIPAA-enabled teams, and their data processing is not covered by the PlatoForms BAA.

PDF completion and delivery

PlatoForms offers two ways to turn an existing PDF into an online form:

  • Web PDF Form: Converts the PDF into a responsive web form that adjusts to phones, tablets, and desktops. Respondents can use a Classic layout or a Conversational, one-question-at-a-time view. Choose this when mobile-friendly completion matters more than matching the PDF layout on screen.
  • PDF Fillable Form: Displays the original PDF in the browser with interactive fields placed over it. Respondents see the document’s original design while filling it out. Choose this when the on-screen layout must stay unchanged; small screens may require scrolling or zooming. This form type supports the Classic layout only.

Both options return a completed PDF using the original document layout. The difference is the filling experience: a responsive web layout or fields over the original PDF. With either option, submitted answers are mapped back to the PDF.

Teams can retrieve completed PDFs from submissions and configure delivery through notifications or integrations. Check field mapping and test a completed submission before sharing a form, especially when using automatic field detection.

Master Form and approval workflows

Master Form links one collection form to multiple PDFs so shared details do not have to be entered again for every document. It is useful for onboarding and application packets.

Workflow moves forms through configured steps so different people can supply information, review, or sign. Master Form handles multiple output documents; Workflow handles participation across steps. They can be used together.

Platform, API, and MCP

PlatoForms is a hosted service managed through a browser. Forms can be shared by link or embedded on a website. The user documentation covers building, publishing, and managing forms.

The REST API supports integrations with forms and submissions. The MCP integration lets supported AI assistants work with PlatoForms using the access granted by the account. HIPAA, BAA, and security

HIPAA compliance is available on Silver and Gold, with a signed Business Associate Agreement (BAA). A Team Admin must request HIPAA enablement; coverage applies to the enabled team’s forms and submission data. See HIPAA setup and scope and the Trust Center.

MCP starts disabled for HIPAA-mode teams. Data sent to an AI provider through MCP is not covered by the PlatoForms BAA, and the public ChatGPT plugin must not be used to process protected health information (PHI). See MCP data boundaries before connecting a healthcare workflow.

Pricing model

PlatoForms offers a 15-day free trial, followed by a per-plan subscription for paid capabilities and capacity. Paid tiers are Bronze, Silver, and Gold, with monthly and annual billing options. Limits vary by plan, including live forms, submissions, storage, team members, and API use.

There is also a limited free plan with one live form and 100 submissions per month. It is not an unlimited free plan. Limitations

  • No unlimited free plan: The free plan has limits on live forms, submissions, and storage. Paid capabilities require a subscription after the trial.
  • Not the first choice for conversational forms alone: PlatoForms supports conversational forms, but its primary fit is a workflow that needs completed PDFs. A conversation-style survey without document output is not its main differentiator.
  • PDF setup still needs review: Automatic detection does not remove the need to check fields, mapping, and completed output.
  • HIPAA is conditional: It requires an eligible plan and team enablement; the BAA does not automatically extend to external services or AI providers.
  • API and MCP are plan-limited: They require appropriate access and are subject to usage quotas.
