Create Web Form from Template

You can speed up your form creation with Web Form templates. Instead of starting from a blank page, you can pick a ready-made template, such as an application, checklist, or survey, and customize it to fit your needs. Browse all 2,000+ templates to find one that matches your use case.
Web Form templates open from Start from a template under Web Form in Start to build. Templates support both Classic and Conversational modes. Document templates also exist: choose Start from a template under PDF Document Creator. The template site offers both.
Create a Web Form from template
Follow these steps:

Log in to your PlatoForms account.

On the Dashboard, click + Start to build at the bottom left.

In the pop-up window, under Web Form, click Start from a template. (The other tiles, Classic Web Form, Conversational Form and Start with AI chat, start without a template; Classic Web Form opens a blank builder.)

The Create from a template window will open. Here you can:
- Create a Classic Form or Create a Conversational Form: the first tiles create a blank Web Form.
- Choose a Template: pick from ready-to-use templates.

Use the options at the top to filter your templates:
- Both / Classic / Conversational - choose your preferred form style.
- Search bar - quickly find a specific template by name or description.
- Types / Industries - browse by category (e.g., Application, Checklist, Survey, Healthcare).

Hover over a template and click Preview Template to try it, or Use this Template to open a copy in the Web Form builder.
