How to Convert a Canva Design into a Fillable Online Form

What you need


Install the PlatoForms app in Canva

Open any Canva design, click Apps in the left toolbar, and search for PlatoForms. Install it, hit Connect, and authorize PlatoForms in the popup. One-time setup — the app stays connected after that.

Export and convert

Open the design you want to convert. In the PlatoForms panel, click the export icon in the top right.

Then click Export and convert design.

Review the PDF export settings and click Export. The default settings work for most cases.

Options with a crown icon (like Compress PDF) require Canva Pro and aren’t needed here.

When you see “Processing complete”, click Open form to continue.

Pick your form type

You’ll choose between two options:

PDF Fillable Form keeps your Canva design exactly as-is, with interactive fields layered on top. If the visual design matters, this is the one.

Web PDF Form rebuilds your form as a clean web form with an editable field panel — better for mobile and easier to customize after the fact.

Let AI do the heavy lifting

Once your form opens in the editor, you’ll see an AI-Powered form recognition banner at the top. Click Start Now.

AI scans your design and detects text inputs, checkboxes, radio buttons, date fields, signature lines — pretty much everything. When the scan is done, click Load Form to apply all detected fields to the editor.

We tested across four Canva form designs — a fitness membership form, a massage therapy intake form, a business order form, and a nail salon client intake form. Results were consistently accurate: text inputs, date fields, Yes/No choices, multi-item checklists, conditional follow-up fields, and even dual signature lines all detected correctly. The nail salon form had 31 fields mapped in under a minute.
